Understand the background relating to substance use to support the development of an action plan.
. Explain the reasons for substance use.
. Explain the specific needs and issues of substance users.
. Describe the range of behaviours that you may experience from substance users.
. Describe the risks substance users may pose to themselves and others.
. Explain how to minimise the risks.
. Explain the implications of mental health issues in relation to substance use.
. Identify commonly used examples of substance misuse jargon/terminology.
Be able to develop an action plan with individuals.
. Provide opportunities for the individual to contribute to the development of the action plan.
. Confirm that the individual understands the information provided.
. Agree the process for reviewing the action plan with the individual.
. Provide an action plan that reflects the current circumstances of the individual.
. Record the action plan according to agreed ways of working.
Be able to review the action plan with individuals.
. Gather and record information relating to individual’s progress.
. Provide opportunities to review the action
plan with an individual.
. Identify with the individual the outcomes that have been met and those still to be achieved.
. Identify and agree the next stages with the individual.
