Learning Outcome - The learner will:		Assessment Criterion - The learner can:
1	Understand how young people may be affected by ‘transition’ in their lives	1.1	Explain the circumstances and type of situations that make them transitional in young people’s lives
1		1.2	Identify signs or concern or distress in young people which may relate to transitional experiences
2	Understand how to provide support to young people during transitions in their lives	2.1	Provide opportunities for young people to explore and discuss significant events and experiences that may impact on them
		2.2	Explain how they can support these young people to manage their transition to reach a positive outcome and build on their strengths
		2.3	Explain the boundaries and protocols that govern own role in supporting young people through transitions and make effective links with others should further support be necessary
		2.4	Comply with legal, organisational and ethical requirements relating to exchange of information
